Responsibilities
  • Work with Registered Nurse Manager to provide timely and efficient identification of patients who need discharge planning.聽
  • Provide assessments of psychosocial impact of illness.
  • Provide direct services including discharge planning for patients needing community resources prior to discharge from the acute care setting.聽
  • Experience in a medical setting is preferred.

Qualifications
  • A current unencumbered Social Worker license to practice in the State of Arkansas is required,
  • BLS within 30 days聽
  • Must be able to communicate effectively, both written and oral.
  • Must possess organizational skills, able to prioritize work.
  • Must possess demonstrated training, knowledge, and skills necessary/appropriate to the adult, and/or geriatric age groups
